This section appears only if the educator has had discipline from the Utah State Board of Education within the past 10 years. Revoked and surrendered licenses are always shown, regardless of age.

• Yellow background: Reinstated license or letter of reprimand
• Red background: Revoked, surrendered, or suspended license

Licensure Information Section

This section displays the educator's license information, including the type of license held, current status, and important dates.

License

This field shows the educator's current or most recent license type.

Licensing Levels: To help other licensing agencies, each definition below identifies the "Stage of Teacher Licensure" met by that specific license. These levels are based on the NASDTEC Interstate Agreement for Educator Licensure.

Current Active License Types

• Professional (NASDTEC Stage 3)
  Standard license for fully qualified educators.
  Valid for 5 years. Can be renewed.
• Associate (Not NASDTEC comparable)
  Temporary license while completing a preparation program.
  Valid for 3 years. In some cases, can be extended for up to 2 more years.
• LEA-Specific (Not NASDTEC comparable)
  Temporary, restricted license requested by a district or charter school.
  Valid for 3 years. Can be renewed.
• SOEP Provider Specific (Not NASDTEC comparable)
  Temporary, restricted license requested by a private school for Statewide Online Education Program (SOEP) only.
  Valid for 3 years. Can be renewed.
• DOPL Audiology/Social Worker/Speech Language Pathologist
  Not an educator license. Person licensed through Utah Department of Occupational and Professional Licensure (DOPL). Listed for state funding and reporting purposes only.

Former License Types (No Longer Issued)

These licenses are expired and no longer valid in Utah. They are equivalent to a Professional license and may be renewed.

• Level 1 (NASDTEC Stage 2) - Valid for 3 years.
• Level 2 (NASDTEC Stage 3) - Valid for 5 years
• Level 3 (NASDTEC Stage 3 or 4) - Valid for 7 years

These licenses were temporary or restricted licenses, no longer valid, and are not renewable.

License or Discipline Status

This shows the status of the educator's license.

• New - First time issued
• Renewed - Renewed after expiration
• Reinstated - Restored after suspension
• Expired - Not valid for employment
• Suspended - Temporarily inactive
• Revoked - Permanently invalid
• Surrendered - Given up during discipline process
• Reprimanded - Formal warning, license still valid

License Dates

• Issue Date - When the license was issued
• Expiration Date - When the license expires (typically June 30)
• First Professional License Date - The first time a professional license was issued. If blank, the person was never professionally licensed. If the date shown is July 1, 2000, it does not reflect the original license issue date. This date was assigned during an electronic system update, and the person was professionally licensed before July 1, 2000.

Credentials Section

Lists all endorsements and license areas showing subjects and specialties.

• Expiration Dates: Most professional licenses and endorsements do not have their own expiration dates. Instead, they stay active as long as the main license is current.
• Restricted Licenses: If your main license is restricted, any license area(s) or endorsement(s) attached to it are also restricted.
• For Other States/Jurisdictions: Only credentials that are unrestricted meet the NASDTEC Stage 2 definition. Restricted or temporary credentials do not meet the Stage 2 definition.

Recommending Institution

Shows the program or institution of higher education that recommended the educator.

• A Utah college or university
• A district or charter school - If Professional: LEA Alternate Preparation Program. Licenses renewed before 2009 may list a school district because of how renewals were processed at that time. Contact USBE Licensing if you have questions or need more details.
• Others Not Detailed - Out-of-state program
• USBE - Alternative Routes to Licensure or CTE pathway

Certificate Type

Indicates the type of license.

• Standard - Regular teaching license
• Online-only - Valid only for online instruction (rare)
• Basic - Should be treated as "Standard"

Certificate Status

Same as License Status (see above section).

Assignment Section

Shows the educator's current employment assignment.

• Percentages reflect the full school year
• End Date 6/30/9999 means the assignment is current
• Typical school year begins 7/1
• Teachers Professionally licensed in Deaf Ed, Secondary Ed, or CTE, may teach outside their endorsed subject(s) for up to 25% of their schedule.
  • Note: This does not apply to Driver Education courses.

District/Charter

• Charter schools are listed as their own district.
• If the district and school name match, the employer is a charter or private school.

State Qualified

Shows whether the educator was properly licensed for the assignment and is qualified for state funding.
